Effective March 2, 2009

  • Official transcripts are $3.00 per copy.  Make checks payable to Chilton Public Schools.  Please allow 5 business days to process.
  • Transcript requests must be accompanied by a completed transcript request form along with payment.
  • Transcript requests must come from the student/former student. Requests from a third party will be processed only with written permission from the student/former student.
  • Students who are 18 or older must be the individual who completes the request form and signs it.
  • Parents requesting a transcript can do so only for their son/daughter if he/she has not reached the age of majority.
  • No transcript requests will be accepted via e-mail.
  • The transcript request form is available here and also at Chilton High School.
