District Forms and More

Prior Years

Wisconsin Administrative Code requires school boards to evaluate the status of nondiscrimination and equality of educational opportunity in the school district at least once every five years. During the district self-evaluation, the district must provide an opportunity for participation by pupils, teachers, administrators, parents and residents of the school district. In addition, the district must prepare a written report of the evaluation which shall be available for examination by residents of the school district. A copy of the School District of Chilton's Pupil Nondiscrimination Self Evaluation Report is provided below for review by the residents of the school district. If you have any questions or concerns regarding this report, please feel free to direct your comments to: Mrs. Susan Kaphingst, Superintendent. Mrs. Kaphingst can be reached at: kaphingsts@chilton.k12.wi.us, or by calling the District Office at: (920) 849-8109.
