Dear Parent or Guardian,
The Healthiest Calumet County Steering Committee has asked all high schools in the county to conduct a student survey about alcohol, tobacco, and other drug issues, as well as questions about health and safety behaviors. Your child is being asked to take an 8 to 10 minute survey at Chilton High school on January 27, 2014. In this way, the county and school districts will be better equipped to apply for state and federal grants. The survey and data analysis has been offered to all county high schools free of charge.
Participation in the survey is voluntary on the part of the student, and we hope you will encourage your child to participate so that his or her opinions can be part of the information gleaned from the survey.
The survey is anonymous. No names or any other identifying information is connected to the answers except for the name of the school.
If you have questions, please contact me. If you wish to view the survey, you may do so at the school office.
